B12 injections are often marketed as a quick energy boost, but the real clinical picture is more specific. Vitamin B12 is essential for red blood cell formation, nerve function, DNA synthesis, and methylation. What B12 actually does in the body

Vitamin B12, also called cobalamin, is required for healthy red blood cell production and normal nervous system function. It helps maintain myelin, the protective coating around nerves, and supports DNA synthesis in rapidly dividing cells. B12 also participates in methylation, the biochemical process that helps convert homocysteine to methionine and supports neurotransmitter, detoxification, and cardiovascular pathways.

That is why true B12 deficiency can feel like more than ordinary tiredness. Patients may experience fatigue, weakness, shortness of breath with exertion, headaches, palpitations, numbness or tingling in the hands and feet, balance changes, tongue soreness, mood changes, or brain fog. Symptoms can be subtle and develop over years because the body stores B12 in the liver. By the time symptoms are obvious, levels may have been declining for a long time.

Who is most likely to be low in B12?

B12 is found naturally in animal foods like fish, meat, poultry, eggs, and dairy. People who follow vegan or strict vegetarian diets are at higher risk unless they use fortified foods or supplementation. But low intake is only one cause. Many patients eat plenty of B12-rich foods and still run low because they do not absorb it well.

Absorption depends on stomach acid, digestive enzymes, intrinsic factor from the stomach, and a healthy terminal ileum in the small intestine. Risk increases with pernicious anemia or autoimmune gastritis, bariatric surgery, Crohn's disease or ileal surgery, chronic GI inflammation, older age, and long-term use of acid-suppressing medications. Metformin is another common contributor; patients using it for diabetes, insulin resistance, or PCOS should have B12 monitored periodically, especially if fatigue or neuropathy appears.

Testing matters before treating fatigue as a B12 problem

Fatigue has many possible causes: iron deficiency, thyroid dysfunction, sleep debt, perimenopause, depression, insulin resistance, chronic inflammation, medication effects, low protein intake, or under-fueling during GLP-1 weight management. B12 can be part of the picture, but it should not be assumed.

A standard serum B12 level is a useful starting point, but borderline results need context. Many labs flag deficiency below roughly 200 to 250 pg/mL, while symptoms can appear in some patients at higher borderline levels. Methylmalonic acid (MMA) is often more sensitive for functional B12 status because it rises when cells cannot use B12 adequately. Homocysteine may also rise, though it is less specific because folate, B6, kidney function, and other factors influence it. This makes IM delivery especially useful for patients with malabsorption risk, medication-related depletion, significant deficiency symptoms, or those who have not responded adequately to oral supplementation. The shot itself is quick, usually under a few minutes, and most patients only notice brief injection-site soreness.

What a B12 shot should and should not promise

A B12 shot is not caffeine, and it should not be sold as a guaranteed instant energy surge. If a patient is truly deficient or functionally low, replenishment can improve energy, cognition, mood, nerve symptoms, and exercise tolerance over time. Some people feel better within days; others improve gradually over several weeks as red blood cell production and neurologic function recover.

If your B12 is already adequate, more is not automatically better. In that case, persistent fatigue deserves a broader evaluation instead of repeated shots. The most clinically useful approach is targeted: test when appropriate, identify why B12 is low, correct the deficiency, and monitor response.
